enhance collapse_vars (#1862)

- extend expression types
  - `a++`
  - `a=x;`
- extend scan range
  - `for(init;;);`
  - `switch(expr){case expr:}`
  - `a = x; a = a || y;`
- terminate upon `debugger;`
